What it is
Community living, supported
The OBRA Waiver funds home- and community-based services so eligible adults can live in the community rather than in an institutional setting. Instead of one-size-fits-all care, it wraps supports around the person and their goals.
As your Service Coordinator, CareFwrd helps you locate, coordinate, and monitor the services in your plan, and stays with you as your needs change.
Who is eligible
Final eligibility is determined by the Commonwealth of Pennsylvania. Generally, the OBRA Waiver serves adults who:
- Have a severe developmental physical disability
- Meet an institutional level-of-care requirement
- Are able to be safely supported in the community
- Meet the program's financial eligibility criteria
