The Way Home exists to make that transition safe, organized, and hopeful, so returning to the community feels possible, not overwhelming.
Who qualifies
Is The Way Home right for you or your loved one?
Every situation is unique, and final eligibility is determined by the Commonwealth. Generally, this path may fit adults who:
- Currently live in a nursing facility and want to return to the community
- Are able to live in a community setting with the right services and supports
- Need help coordinating housing, services, and providers to make the move
- Have family or caregivers who want to help make community living work
