What it is. Life Moves Fwrd™ is CareFwrd's specialized Service Coordination for adults living with traumatic brain injury — pairing everyday support with a deep understanding of how brain injury changes daily life.
Who it's for. Adults recovering from or living with a TBI who need help navigating services, providers, and the systems around their care.
How CareFwrd helps. We coordinate the right supports, remove barriers, and advocate at every step — always at the client's own pace.

The OBRA Waiver supports eligible adults with severe developmental physical disabilities so they can live and stay active in their communities rather than in an institutional setting.
As your Service Coordinator, we help you locate, coordinate, and monitor the home- and community-based services that keep you supported and independent.

Act 150 Program
The Act 150 Program helps qualified adults access long-term services and supports, often with a cost-sharing component based on income.

The OBRA Waiver serves eligible adults with severe developmental physical disabilities who meet Pennsylvania's criteria and would otherwise require an institutional level of care. Final eligibility is determined by the Commonwealth — we're happy to help you understand the process.
