Service Coordination
In Pennsylvania, care navigation is delivered as Service Coordination. Your Coordinator is your point of connection to the services and supports in your plan. We locate the right providers, coordinate everyone involved, and monitor how things are going, adjusting and advocating as your needs change. You always keep control of your own choices.
